Closing the Gaps in Diabetes Care

November is Diabetes Awareness Month, which is a reminder that behind every statistic is a person navigating the daily realities and challenges of diabetes. This makes diabetes one of the most widespread chronic health conditions in the country and a major public health challenge.1 Despite many medical advancements and new therapies, many patients still face real challenges like managing complex medication regimens, making lifestyle changes, and needing ongoing medical support. 

These barriers often lead to preventable complications and higher healthcare costs, but more importantly, they can leave patients feeling overwhelmed and isolated. That’s why improving diabetes care requires a more connected, collaborative approach across healthcare partners.
